ABSTRACT

@#AIM:To evaluate the efficacy and safety of dexamethasone intravitreal implant for the treatment of noninfectious refractory uveitis and analyze its influence on application and dose of systemic corticosteroids and immunosuppressive agent.<p>METHODS: This was a retrospective, case series study conducted between January 2018 and September 2019, the patients with noninfectious refractory uveitis and treated with intravitreal implantation of dexamethasone intravitreal implant were included. The best corrected visual acuity(BCVA), central retinal thickness(CRT), vitreous haze score, and intraocular pressure(IOP)at 1mo, 2mo, 4mo and 6mo after the implantation of dexamethasone intravitreal implant were compared with the baseline. Meanwhile, the dosage of corticosteroids and immunosuppressive agent used before and after the implantation of dexamethasone was also evaluated. The data was displayed as mean±standard deviation and analyzed using the paired <i>t</i>-test.<p>RESULTS: A total of 19 patients(21 eyes)were included in the present study. The mean baseline BCVA of the included patients was 0.671±0.469, while the mean CRT was 369.667±177.100μm, and the mean intraocular pressure(IOP)was 14.252±3.181mmHg. One month, 2mo, 4mo and 6mo after the injection of Ozrudex, BCVA was significantly improved compared with the baseline(<i>P</i><0.05), while the CRT and vitreous haze score were significantly reduced(<i>P</i><0.05,<i> P</i><0.05). During the follow-up period, IOP of several patients significantly increased, but normal IOP can be observed at the end of the follow-up for all included patients. At baseline, 6 patients(6 eyes)were treated with systemic corticosteroids with an average dosage of 35.8mg/d, while only 2 patients were treated with systemic corticosteroids at the end of the 6mo follow-up(5mg/d). Apart from 3 eyes with Behcet's disease which showed recurrence of macular edema between 4-6mo, no other patient underwent that. <p>CONCLUSION: Dexamethasone intravitreal implantis considered to be a safe and effective approach for non-infectious refractory uveitis which can reduce systemic medication, with significant improvement of BCVA, reduction of CRT, vitreous haze score and the dosage of systemic corticosteroids.

?AIM:To analyze the morphological changes of macular soft drusen and drusenoid pigmental epithulium detachment ( DPED ) after subthreshold micropulse laser treatment ( SMLT) .?METHODS: Fourteen patients ( 20 affected eyes ) with soft drusen and DPED clinically confirmed from August 2016 to October 2018, were included in this study. 577 nm yellow laser of SMLT was applied for soft drusen and DPED. The changes of soft drusen and DPED in best corrected visual acuity ( BCVA ) ( LogMAR ) and height, diameter and cross-sessional area according to fundus autofluorescence and SD - OCT examinations were observed after SMLT.?RESULTS: BCVA was not significant difference after treatment of soft drusen (P=0.260), and the DPED (P=0. 736 ) than that of the baseline. Compared with the baseline values, the height and cross-sessional area of soft drusen were reduced at the 6mo after treatment ( P=0. 008, P=0.034) . Compared with the baseline values, the differences were not statistically significant in height, diameter and cross - sectional area of DPED after treatment.?CONCLUSION: BCVA was not reduced for drusen and DPED after SMLT, however, the height and cross -sessional area of soft drusen was reduced compared with those before treatment, and the differences were not statistically significant in height, diameter and cross -sectional area of DPED before and after treatment. The results indicated that SMLT was effective for soft drusen, but was not effective for short-term treatment of DPED. SMLT caused no damage to the visual acuity in treatment of soft drusen and DPED, but prospective, controlled, large sample and long-term follow-up studies should be required.

ABSTRACT

After implementing a series of prevention and control strategies of Corona Virus Disease 19(COVID-19), China′s local epidemic situation has been basically blocked.While China has achieved a periodic success, global pandemic situation is still serious.Together with the timeline of China′s epidemic prevention and control this study reviewed the main strategies and measures in response to COVID-19 epidemic from Dec.2019 to Mar.23, 2020, and summarized China′s prevention and control plans and experiences.We hope all countries could fully understand the importance of non-pharmaceutical public health interventions, make positive and effective adjustments to the measures and strategies of prevention and control based on their own national conditions.

ABSTRACT

The digital intelligent medical technology such as three dimensional visualization, three dimensional printing,enhanced/virtual reality technology, indocyanine green molecular imaging are widely used in the clinical diagnosis and treatment of hepatobiliary and pancreatic diseases and showed a booming trend. Perihepatic hilar biliary tract diseases with complex anatomical structure, special physiological pathology character and lower efficiency of diagnosis and treatment are the major and difficult disease in the field of surgery. Efficient preoperative evaluation is the key to improve the efficiency of diagnosis and treatment. The digital intelligent medical technology can be used to obtain the comprehensive, individualized image diagnostic information,and comprehensive analysis the anatomy relations of the hepatic artery, portal vein, bile duct and lesions, and quantitatively measure the liver volume, formulate the reasonable operation scheme, determine the best approach of surgery, predict intraoperative injury risk and improve the surgical treatment effect.

ABSTRACT

<p><b>OBJECTIVE</b>To construct and evaluate the criteria of injury based on the theoretical model that 'Injury' is the combination of 'Hurt' and 'Harm', in order to provide theoretical basis for scientific and rational research on injury in future epidemiological survey.</p><p><b>METHODS</b>The criteria of epidemiological injury was consected by analyzing documents and consulting experts, then Delphi method was used to evaluate its rationality.</p><p><b>RESULTS</b>The epidemiological definition for 'Injury'is the combination of 'Hurt' and 'Harm'. 'Hurt' means the wound in clinical medicine, while 'Harm' means the damage from the views of public health. The active coefficients of two rounds Delphi Method were both 100%. The authority coefficients of two rounds were 0.88 and 0.87, respectively. The coordination coefficients were 0.44 and 0.62 (P<0.05), respectively. After two rounds consulting, all scores of rationality for definition of 'Hurt', definition of 'Harm', 'Injury' being the combination of 'hurt' and 'harm', 'Injury' should be graded, Graded based on the 'Injured' time were above 3, and all values of CV for those index were below 0.25 except Graded based on the 'Injured' time.</p><p><b>CONCLUSION</b>The rationality of Definition of 'Hurt', Definition of 'Harm', 'Injury' being the combination of 'hurt' and 'harm', 'Injury' should be graded and Graded based on the 'Injured' time is good. There is still a certain degree of divergence about Graded based on the 'Injured' time.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To accomplish the computation of Fisher exact probability test for fourfold table data in Excel.</p><p><b>METHOD</b>The computing program of exact probability method for medical data in fourfold table design was edited by employing the IF statement and the relevant functions, such as SUM, FACT, DSUM, etc in Excel. The computational results are compared and evaluated according to the case studies.</p><p><b>RESULTS</b>The output of Fisher Exact Probability was generated and presented correctly following the input of four numerical values into the computation program in the setting of Excel. The parametric outcomes are in agreement with those produced by SAS and SPSS, in the combination tables containing the P value, two-tailed cumulative P value, left-tailed P-value, right-tailed P-value, Chi2 values and P values both for direct Chi-squared test and corrected Chi-squared test.</p><p><b>CONCLUSIONS</b>Direct Chi-squared test, corrected Chi-squared test combined with Fisher Exact Probability test for fourfold table data can be conveniently, rapidly, and accurately accomplished in Excel.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To explore the etiological factors of hand special chronic infections and their relationship with tuberculosis, and to give evidence for clinical diagnosis as well as treatments.</p><p><b>METHODS</b>From 2002 to 2004 pathologic inspection, acid-fast stain, bacterial cultication, mycobacterial cultivation were performed in all 29 cases of hand special chronic infections.</p><p><b>RESULTS</b>All cases showed granulomatous lesions in pathological appearance, 2 positive in acid-fast stain, 12 positive in bacteria cultivation, and 1 nocardiosis, 1 staphylococcus epidermidis, 7 M.marinum, 1 M.tuberculosis, 1 M.fortuitum, 1 M.kansasii.</p><p><b>CONCLUSIONS</b>Non-tuberculo-mycobacterium (NTM) especially M.marinum are far more important as the major factor than tuberculosis and other bacterial in hand special chronic infections. Bacteria cultivation should be routine examined for all cases.</p>

ABSTRACT

SMV is one of main diseases of soybean, which could affect yields and quality of soybean seriously. It was effective to soybean breeding by studying the expression of resistant gene to SMV with molecular technology. In this study, a soybean resistance line, DongNong 8143, was used to construct a subtractive cDNA library by SSH from soybean leaves inoculated by SMV No.1 at primary stage. cDNA dominantly or specifically expressed in infected leaves was purified using PCR Purification Kit and cloned into pGEM-T easy vector. Colonies were grown on LB-agar plates containing ampicillin, X-gal and IPTG. A subtractive plasmid library was constructed by SSH. Then the library was transformed to host bacteria E. coli DH5alpha, and the titer of the library was measured as 2 x 10(3) . 64 clones were picked up randomly and sequenced. Of them there is 50 clones which result of sequenced were good. The length of EST fragment varied from 136bp to 691bp, and the average length is 456bp. Among them, 41 sequences has poly(A). Through ESTs were compared with sequences in unigene database of GeneBank with BLASTn and BLASTx algorithm, 38 ESTs of them had comparatively clear results and the percent of them in acquired ESTs is 74%. The EST expression profile showed that the resistance-related genes include cell protection, signal transduction, restrict pathogen growth, system acquired resistance, and house-keeping gene. There are 12 ESTs, which have not comparatively clear results, that maybe new genes.

ABSTRACT

<p><b>OBJECTIVE</b>To evaluate the influence of the surveillance system and preventive measurements on the control of severe acute respiratory syndrome (SARS) in a university in Guangdong Province.</p><p><b>METHODS</b>A university with more than thirty thousand undergraduates, staff and their relatives was retrospectively studied, from which information regarding the status of epidemic, organization of leadership, disease control strategies and measures were collected and analyzed.</p><p><b>RESULTS</b>The construction of the surveillance system in such a model as "individual-dormitory/home-class/unite-faculty and institute-university" largely contributed to the achievement of the goals of low incidence, no secondary, no epidemic, and no death. A series of control measures benefited the early diagnosis, effective isolation, prevention, and treatment of SARS control.</p><p><b>CONCLUSION</b>SARS could be effectively controlled in university only if strict surveillance system is built up, and all-round preventions, including early isolation of both confirmed or suspected cases and close contacted persons, are carried out.</p>

ABSTRACT

In this paper, metabolic networks of the Corynebacterium glutamicum GWY020 and the two de-rivatives carrying additional mutations HUI821and GUI089 were established and modified. The concentra-tions of extra-cellular metabolites were determined under sub-steady-state (50 h~52 h) of the batch culture. The metabolic flux distribution maps of the three strains were obtained, compared and analyzed. These re-sults indicate that the introduction of analog supersensitive marker or analog resistant marker skew the metabolic flux towards the formation of L-Arginine. This study revealed the usefulness of the metabolic flux analysis as a tool for verification of existing production strains. The analysis may play an important role in helping us to rationally re-design metabolism for further improvement of fermentation process.

ABSTRACT

A L-histidine producing mutant was derived from Corynebacterium glutamicum HZ4221(TRA R DCP R AMT R histidase - )by means of mutagenesis with N-methy-N′-nitro- N-nitrosoguanidine(NTG).Contrast to original strain,the amount of histidine accumulation reached to a level of 5.31g/L in a medium containing 80g/Lglucose and 30g/L ammonium sulfate after cultured for 72 hours; the transketolase activity reduced to a degree of 15.7%.The utilization of the carbon sources,genetic stability,effect of metal ions were also been investigated in this paper.
